Officer Agreement Form

Each year the candidates running for chapter office should complete the Offcer Agreement Form and submit the form to the chapter's Nominating Committee.  After elections are held, all incoming officer forms should be forwarded to Martha Montague,  the Chapter Resources Manager,  via fax (336-492-5831) or by email to mmontague@cmaanet.org.
